Colorectal Diagnostic Assessment Program (DAP)

The Colorectal DAP improves access to timely diagnostic testing and a coordinated approach to patient care supported by a multidisciplinary team of radiologists, surgeons and other key healthcare providers. The program streamlines the treatment and care of patients who have been diagnosed with colon or rectal cancer (and referred to Oakville Trafalgar Memorial Hospital) and provides them with support from an emotional and educational standpoint.

What You Can Expect from Us

  • A patient navigator (who is a staff member) helps to coordinate and book all of your appointments
  • Once all necessary tests are completed, an appointment will be set-up for you to meet and discuss your results with your surgeon
  • After receiving a completed referral form from your physician, we aim to contact our new patients in a timely manner
  • Our team will provide you with support and education
  • The DAP staff will inform your family doctor of your results and appointments, and keep them up-to-date about your care

Prior to Your First Appointment

  • Your first visit with us will be about one hour in length and you will meet your patient navigator, be provided with helpful materials and have your blood work completed
  • It is recommended that you bring someone with you. If you use a cane, walker, or wheelchair, please bring it with you
  • Parking rates and hospital maps can be found on our website, and our volunteers will help you find your way when you are here

Your Appointment & What to Bring with You

  • Your Ontario Health Card. If you have the red and white card, please bring a piece of photo identification, driver’s license or passport
  • List of prescription medications
  • List of non-prescription medications such as vitamins, minerals, herbal supplements, and any other natural health products
