Mississauga Halton Diabetes Foot Care Program
The Mississauga Halton Diabetes Foot Care Program provides foot care education and management to individuals with diabetes at moderate to very high risk for a diabetic ulcer or amputation who have financial barriers to accessing fee-for-service foot care. The level of risk is determined by the In-Low's 60-second Diabetic Foot Screen.
The program does not provide ongoing nail care and may be time limited.
Care is provided by a Chiropodist or a Registered Practical Nurse, with advanced foot care certification, at various community locations within the former Mississauga Halton Local Health Integration Network.
The program offers a number of evidence-based interventions clinically focused on prevention including:
- Diabetic foot care
- Offloading devices & orthopedic shoewear
- Footwear assessment
- Gait assessment
Referrals
There are two ways to be referred to the program:
1. Adult Diabetes Programs of the former Mississauga Halton Local Health Integration Network
2. Home and Community Nursing Clinics
All referrals are to be submitted to the Central Intake Program for review and triage.
Cancellation & Failing to Attend Appointments
The Mississauga Halton Diabetes Foot Care Program is committed to providing care to active patients that regularly attend their appointments and are engaged in their care plan. For that reason, we would appreciate at least 24 hours notice of cancellations. You may be discharged from the program for the following reasons if there are no extenuating circumstances.
- Repeatedly not attending your scheduled appointments
- Repetitive appointment cancellations
- No appointment attendance within a year
