The Breast Diagnostic Assessment Program (DAP) is a clinic where breast imaging results are reviewed by a multidisciplinary team, consisting of radiologists, surgeons and other healthcare providers. In the DAP:

• A patient navigator (staff member) helps to coordinate and book all of your appointments.
• A patient navigator provides you support and education.
• Patients see both the patient navigator and a surgeon, to discuss their breast imaging at their clinic visit.

The DAP staff will inform your family doctor of your results and appointments, and keep them up-to-date about your care.

What do I need to know prior to my appointment?

• Your visit will take about 30 mins.
• It is recommended that you bring someone with you. If you use a cane, walker, or wheelchair, please bring it with you.

• Parking rates and hospital maps can be found on our website

• A reminder that we are a smoke-free and scent-free environment. Please do not wear cologne, or scented products while in hospital.

What do I bring to my appointments?

• Your Ontario Health Card. If you have the red and white card, please bring a piece of photo identification such as a driver’s license or passport.
• List of prescription medications.
• List of non-prescription medications such as vitamins, minerals, herbal supplements, and any other natural health products.

What does it mean if I need more tests after my mammogram and/or ultrasound?

• Some women may need further follow-up testing, such as a biopsy. A patient navigator will contact you prior to your biopsy to help answer any questions you may have.
• It’s helpful to know that most women that need more breast imaging tests after a mammogram and/or ultrasound do not have breast cancer.
• If you require additional tests after your 2nd mammogram or ultrasound, you will have an appointment booked with a breast surgeon at the DAP.
• At the appointment with the breast surgeon, you will discuss the results of your breast imaging tests.
