Laboratory Accreditation

QMP–LS/IQMH (Institute for Quality Management in Healthcare) accredits diagnostic medical laboratories for which the ISO 15189:2012 “Medical Laboratories – Particular requirements for quality and competence” standard is applicable. In Ontario, all licensed medical laboratories are required to meet accreditation requirements as a condition of licensure.

Halton Healthcare is required to maintain separate licenses for each of the laboratories at each hospital. As a result, each of these hospitals are required to be assessed by IQMH separately. IQMH’s accreditation process includes regular accreditation assessment visits every four years and self-assessments that occur midway between visits.

Halton Healthcare went “under the microscope” the week of February 6-10, 2017 during the accreditation of its Laboratory Services at Georgetown Hospital (GH), Milton District Hospital (MDH) and Oakville Trafalgar Memorial Hospital (OTMH). The Institute for Quality Management Accreditation Team comprised of seven assessors examined whether the policies, procedures and practices, at our three Hospitals, were in conformance to mandatory IQMH requirements.

The assessment confirmed that we have an excellent Laboratory Quality Management System in place that has been sustained and improved since the last Ontario Lab Accreitation on-site assessment 4 years ago. 

Amongst the many positive verbal comments, Halton Healthcare was commended for it quality laboratory service, very knowledgeable and experienced laboratory staff, an ”awesome” safety program, and excellent working relationships with nursing, management and other clinical areas.
