Halton Healthcare is honoured to have been recognized for our excellence and innovation in clinical care as well as our achievements in building world-class healthcare facilities. Notable achievements include:

National Surgical Quality Improvement Program – Meritorious status (2023)

Halton Healthcare’s Oakville Trafalgar Memorial Hospital has been recognized by the American College of Surgeons National Surgical Quality Improvement Program for achieving “Meritorious” status for surgical patient care in 2022. The goal of the program is to reduce infection, illness or death related to surgical procedures and provide a foundation for surgeons to apply the best scientific evidence to the practice of surgery.

Service Delivery – Award of Merit (2022)

The Canadian Council for Public-Private Partnerships has recognized Oakville Trafalgar Memorial Hospital for demonstrating the resiliency of the P3 model, with top service delivery.

BOMA BEST® Gold Certification (2022)

In October 2022, after submitting more than 200 pieces of evidence and a detailed verification process, Milton District Hospital achieved BOMA BEST® Gold certification from the Building Owners and Managers Association of Canada (BOMA Canada). The hospital, which underwent a significant expansion in 2017, achieved BOMA BEST® Gold certification with an assessment score of 88.5%. BOMA BEST® buildings are certified within five levels; Gold is the second highest level of achievement. Further to this certification, Milton District Hospital went on to win the BOMA Best Award in the healthcare category in May 2023. Followed by BOMA Canada’s BOMA BEST Award in September 2023, celebrating the hospital’s high scoring achievement in each of our certification asset classes.

Canada’s Most Admired Corporate Culture Award (2021 - 2023)

Halton Healthcare has been recertified for the Canada’s Most Admired Corporate Cultures Award program. As a 2021 winner, we are proud to be recognized for this award for the third year in a row, having demonstrated our commitment to preserving and strengthening our corporate culture this past year.

National Earth Award – Health Care Facility (2021)

After achieving the Building Owners and Managers Association (BOMA) Toronto Best Gold Standard earlier this year, EllisDon Facilities Services at Oakville Trafalgar Memorial Hospital and Halton Healthcare have been awarded the 2021 National Earth Award in the Health Care Facility section. The BOMA Canada Earth Awards recognize excellence in resource preservation and environmentally sound commercial building management. Only those who are BOMA Best Certified are eligible for both the Earth and The Outstanding Building of the Year Awards.

In November, EllisDon Facilities Services at Oakville Trafalgar Memorial Hospital and Halton Healthcare were awarded the 2021 National Earth Award – Health Care Facility. Entrants for both the Earth and The Outstanding Building of the Year Awards must also be BOMA BEST Certified.

In June, EllisDon Facilities Services at Oakville Trafalgar Memorial Hospital and Halton Healthcare were awarded the 2021 BOMA Toronto Earth Award – Health Care Facility. BOMA Toronto's Earth Award recognizes buildings and their teams for excellence in environmental management.

Ingenious Award (2018)

Halton Healthcare has been awarded the Information Technology Association of Canada’s (ITAC) 2018 Ingenious Awards in the category of large public organization. Ingenious Awards celebrate the use of technology to improve the performance of large and small businesses, large and small government operations and not-for- profit organizations.

Silver Infrastructure Award (2018)

The Canadian Council for Public-Private Partnerships has recognized the Milton District Hospital expansion project for its value for money, design, technological innovation and the role the hospital is expected to play in empowering medical excellence.

Leadership in Energy and Environmental Design (LEED®) Gold (2018)

The Canada Green Building Council has awarded Halton Healthcare its second-highest rating for excellence in sustainability, efficiency, environmental quality and innovation in design for the Milton District Hospital expansion. Accreditation with Exemplary Standing (2018) Following our most recent Accreditation Survey in May 2018, Halton Healthcare was awarded Accreditation with Exemplary Standing, by Accreditation Canada. Accreditation with Exemplary Standing is the highest level of performance granted.

One-Link - Partnering for a Healthy Community Awards (2018) Recipient: Pinnacle Award

The Mississauga Halton LHIN’s one-Link enables equitable access and coordinated care to make it possible for people to receive the right addiction and mental health care, at the right time and in the right place. Since implementation in the fall of 2014, one-Link has processed over 41,000 referrals for individuals 16 years of age and over, seeking support for mental health and addiction concerns. Through the application of standardized processes developed through Continuous Quality Improvement at all levels of the model, one-Link has established its operations in ensuring that screening and referral coordination is completed with standardized processes, daily attention to wait times, and minimizing work flow that does not add value to clients or families who are referred for services. Ongoing collaboration with the 10 Mississauga Halton funded Addiction and Mental Health Service providers, with oversight from a Steering Committee, ensures that the vision of one-Link is operationalized each and every day.

2017-2018 Advanced Gerontological Education (AGE) Recognition Award with distinction (category of Acute Care/Hospital) – October 30, 2018

The award was granted to Halton Healthcare for providing staff knowledge, skills and confidence in caring for patients with dementia. The award was received from AGE for Halton Healthcare’s commitment to Gentle Persuasive Approaches (GPA) education, an innovative dementia care education curriculum based on a person-centred care approach.

Established in 1992, NICHE is North America’s leading nurse-driven program designed to help hospitals and healthcare organization improve the care of older adults.

Consulting Engineering Award of Excellence (2016)

The Consulting Engineers of Ontario recognized Oakville Trafalgar Memorial Hospital for its outstanding power distribution system. Smart Energy Design Incentive Award (2016) Ontario’s Independent Electricity System Operator acknowledged the innovations made at the new Oakville Trafalgar Memorial Hospital to enhance energy efficiency. Hardsurface Award (Terrazzo) (2016) The Tile and Marble Association of Canada, representing leading members of the construction industry, commended Halton Healthcare for the thoughtfully-designed hard surfaces installed at the new Oakville Trafalgar Memorial Hospital.

Central Intake - Partnering for a Healthy Community Awards (2016)

Recipient: Access Creating Simplified and Equitable Access to Diabetes Care The Mississauga Halton Central Intake Program was launched in 2013 to establish a single access point for all diabetes programs in the Mississauga Halton LHIN. The program is funded by the LHIN and operated by Halton Healthcare.

The Central Intake Program worked in collaboration with all LHIN-funded diabetes programs to improve access and care coordination across the region. The initiative included implementation of a single referral form, standardized insulin order, standardized regional triage criteria and a wait time tracking mechanism. Having proven its success in streamlining the referral process and establishing equitable access for diabetes services, the LHIN provided additional funding to Halton Healthcare to expand the scope of Central Intake to manage referrals for diabetes foot care, addiction and mental health services in 2014.

In 2014, the program developed and implemented an eReferral solution to improve efficiency and transparency in managing the growing volume of referrals across multiple pathways. The eReferral was first deployed for diabetes services and later expanded to addictions and mental health services in 2016. The eReferral allows seamless flow of the referral between referral sources, Central Intake and partnering services. It enables live referral status updating and tracking to all parties and system-wide reporting. The collaborative efforts and success of the Central Intake Program and the diabetes programs in establishing simplified and equitable access has been sought as a model by many other LHINs across the province.

Partnering for a Healthier Tomorrow Awards (2015) Recipient: Accessible and Sustainable Health Care

Mississauga Halton Diabetes Foot Care Program The Mississauga Halton Diabetes Foot Care Program provides preventative foot care services to high risk individuals with diabetes who have significant barriers to using existing fee-for-service programs. Working to minimize all access barriers, clinicians provide services in 11 clinic locations including: five in Mississauga, one in Halton Hills, one in Acton, two in Milton and two in Oakville, leveraging space of existing community agencies.

The establishment of these foot care clinic sites has fostered the development of new health hubs at these locations, with other health care services coming on board, resulting in a menu of services all available at one community location.

Since implementation in June 2014, services have been provided to more than 750 patients. Results indicate improved outcomes and a decrease in patient risk for foot care complications following treatment. This community partnership model has proven to generate great satisfaction among patients and host agencies.
