Halton Healthcare Wins LHIN Award


On September 26, 2016, Halton Healthcare’s Central Intake Program received the “Creating Simplified and Equitable Access to Diabetes Care” award at the 2016 Partnering for a Healthy Community awards, hosted by the Mississauga Halton Local Health Integration Network (LHIN).

Our Mississauga Halton Central Intake Program was established in 2013 in partnership with Trillium Health Partners. The program receives, triages and directs referrals to regional healthcare providers for diabetes care, mental health services and foot care.

The program created a single access point for all diabetes programs in the Mississauga Halton LHIN, improving access and care coordination across the region. The initiative implemented a single referral form, standardized insulin orders and regional triage criteria, and a wait-time tracking mechanism.

An eReferral solution was introduced to improve efficiency and transparency in managing the growing volume of referral across multiple pathways. Since the launch of eReferral, the program has processed over 25,000 referrals for the different referral pathways and more than 1,750 referral sources have adopted the model.

The collaborative efforts and success of the Central Intake Program and the diabetes programs in establishing simplified and equitable access serves as an example for other LHINs across Ontario.

The Partnering for a Healthy Community awards honour collaborative initiatives underway among health service providers across our LHIN that are benefitting patients, clients, families and residents.
